Collection of personal information

You can access many of the pages on the InforMED website without giving us personal information. However, to save treatment information and notes from a patient decision aid you will be asked to register and provide some personal information. The types of information that we collect may include:

• age and gender
• your responses about skin type, values and trade-offs
• your responses about decisions
• your personalized results page
• your notes
• comments or feedback through our submission form

Use of personal information

By visiting the InforMED website and giving over personal information you are implicitly consenting to the use of your information. We shall use any personal information you give to us in accordance with this policy, and with any additional statements appearing on forms used for submitting your personal information. We shall not disclose your personal information to any third parties without obtaining your prior consent unless we are required by law to do so. In particular:

• If you use an online patient decision aid: each patient decision aid allows you to create a personal record of your journey by building a summary of the pages you visit and your answers to all the questions asked. This summary will be printable, if you chose to print it for your record or to share with others, such as family or a health professional. It can be used when discussing your treatment preferences.
• If you request to deactivate your account or delete your personal information, we will endeavor to fulfill your request but some personal information may persist in backup copies for a certain period of time and may be retained as necessary for legitimate research and/or business purposes or to comply with our legal obligations.
• If you contact us regarding the website or to give feedback: we will use your details to reply to you. We try to respond to all feedback where a valid email address is included. If you make a comment or complaint about other aspects of the service, such as the patient decision aids, we may use your details to investigate your comments.
• With your permission, academic institutions may occasionally use our data in studies. In these instances, your agreement would always be sought in advance in line with the Personal Information Protection and Electronic Documents Act and Windsor Clinical Research Inc.’s confidentiality policy. You will have the choice not to participate in such a study. The information used for such research would always be made anonymous and will be confidential.
• We may also need to disclose your personal information to respond to or comply with any law, regulation, subpoena, court order or other legal obligation.

Information Collected Automatically

InforMED also automatically collects, through our servers and the use of cookies (small text files sent to your computer), web beacons (also known as clear or single pixel .gifs) and other technologies, information that tells us how you use the Service, such as:

• Internet Protocol ("IP") address used to connect your computer to the Internet;
• Computer, device and connection information, such as browser type and version, operating system, mobile platform and unique device identifier ("UDID") and other technical identifiers;
• Uniform Resource Locator ("URL") click stream data, including date and time stamp, referring and exit URLs, search terms you used, and pages you visited or searched for; and
• For location-aware services, the physical location of your device in order to provide you with more relevant content for where you are in the world.

We may use and disclose information that does not reveal your identity or permit direct association with any specific individual, such as browser and device information, anonymous usage data and aggregated information, for any purpose, except where we are restricted by law. If we combine non-personal information with personal information, the combined information will be treated as personal information for as long as it remains combined.

Data storage

All data obtained by the InforMED Website and patient decision aids is held and used in compliance with the Personal Information Protection and Electronic Documents Act.

InforMED takes precautions to safeguard your personal information against loss, theft and misuse and unauthorized access, disclosure, alteration and destruction through the use of appropriate administrative, physical and technical security measures. Despite these measures, InforMED cannot guarantee that these events will not occur.
